At Renewed Vision Counseling and Developmental Disability Center, our Consultative Assessment Services provide comprehensive evaluations that help individuals, families, and support teams better understand a person's strengths, support needs, and opportunities for greater independence. These assessments serve as the foundation for developing effective, person-centered services that improve quality of life and long-term outcomes.

Our assessments are conducted by qualified professionals with expertise in intellectual and developmental disabilities, behavioral health, and person-centered service planning. We evaluate areas such as functional abilities, adaptive living skills, communication, social development, behavioral support needs, community participation, environmental factors, and overall support requirements.

Rather than focusing only on challenges, we take a strengths-based approach that identifies existing abilities while recognizing barriers that may be limiting an individual's success. By gathering information from the individual, family members, caregivers, direct support staff, and other professionals, we develop a complete picture of the person's needs and provide practical, evidence-informed recommendations that can be implemented across home, school, work, residential, and community settings.

Renewed Vision works collaboratively with case managers, guardians, healthcare providers, schools, and other service providers to ensure assessment findings support Medicaid waiver requirements, Individual Service Plans (ISPs), and other care planning processes. Our recommendations are designed to improve service coordination, promote consistency among providers, and help individuals achieve meaningful and measurable outcomes.
