Continuous Home

At Renewed Vision Counseling and Developmental Disability Center, our Youth and Adult Continuous Home services provide safe, supportive, and structured residential environments where individuals can develop the skills necessary to live healthy, independent, and fulfilling lives.

A Continuous Home is a community-based residential service that offers 24-hour support to children, adolescents, and adults with intellectual and developmental disabilities, behavioral health needs, or other functional challenges who require ongoing assistance with daily living. Our homes are designed to provide stability, promote personal growth, and create opportunities for individuals to become active and valued members of their communities.

Renewed Vision's dedicated residential team provides individualized support tailored to each person's unique strengths, goals, and level of need. Services may include assistance with personal care, medication management, meal preparation, household responsibilities, communication, social skills development, behavior support, transportation, community integration, recreational activities, medical appointment coordination, and independent living skill development.

Our approach is centered on dignity, respect, and person-centered care. Every resident participates in developing goals that reflect their interests, preferences, and aspirations. Staff provide encouragement and skill-building rather than simply completing tasks for individuals, helping residents increase independence and confidence while maintaining their health and safety.

We collaborate closely with families, guardians, case managers, healthcare providers, therapists, schools, employers, and other members of each individual's support team to ensure coordinated, high-quality care. Through ongoing communication, individualized planning, and continuous progress monitoring, we help individuals achieve meaningful outcomes while enhancing their overall quality of life.
