Supported Family Living

At Renewed Vision Counseling and Developmental Disability Center, our Supported Family Living (SFL) services empower individuals with intellectual and developmental disabilities to live safely and successfully in their own home or with their family while receiving the individualized support they need to achieve greater independence and an improved quality of life.

Supported Family Living is a person-centered service designed to provide flexible, individualized supports based on each person's unique goals, abilities, and daily living needs. Rather than removing individuals from their natural support system, SFL strengthens their ability to remain in the least restrictive environment while developing the skills necessary to become as independent as possible.

Renewed Vision works closely with individuals, families, guardians, and support teams to create services that reflect each person's preferences, strengths, and long-term goals. Our trained Direct Support Professionals provide assistance with daily living activities such as personal care, meal preparation, medication reminders, budgeting, household management, communication, transportation, community participation, social skill development, and independent living skills. Services are tailored to promote growth while encouraging individuals to make their own choices and actively participate in everyday life.

Our staff also provide coaching, education, and ongoing support to family members and caregivers, helping them build effective strategies that foster independence while maintaining a safe, stable, and supportive home environment. We believe families are valuable partners in the support process and work collaboratively to ensure everyone involved is equipped to help the individual succeed.

Renewed Vision coordinates services with case managers, healthcare providers, therapists, schools, employers, and other members of the individual's interdisciplinary team to ensure supports remain consistent and aligned with each person's Individual Service Plan (ISP) and Medicaid waiver requirements. Through regular communication, progress monitoring, and person-centered planning, we continuously adjust supports to meet changing needs and celebrate meaningful achievements.
